Parents have the opportunity to voluntarily provide their child(ren)’s COVID-19 vaccination documentation to the school nurse. Providing this information to the school district is completely voluntary

In accordance with current public health guidance, vaccinated students who are asymptomatic do not have to quarantine if they come in close contact with an individual who is COVID positive. Therefore, having this information will help the district with the contact tracing process.

In order for the information to be submitted, you must have a personal Google account or use your child’s district Google account. The information will be kept confidential
