Athletics Registration

Registration Instructions

Registrations for participation in the 2019-2020 Athletics Program at Timothy Edwards Middle School are processed online through Ecollect Forms in Parent Portal. Follow the steps below to begin the registration process:
  1. Sign into your Parent Portal account Click Here
  2. If you have more than one child in the school system, select the appropriate child at the top
  3. Click on “Ecollect Forms” in the left hand column
  4. Select the “TEMS [Fall/Winter/Spring] Athletic Registration” link; complete the form and submit
  5. Select the “Parent Concussion Education Consent” link; review all information, complete the form, and submit
  6. Select the “Student & Parent Cardiac Arrest Plan and Consent Form” link; review all information, complete the form and submit

A current Physician’s Statement and Sports Participation Fee must also be submitted to the TEMS main office by the registration deadline listed below.

 2019-2020 Physician's Statement Form
  •  Required only once per academic year
•  Must be dated after June 1, 2019
Sports Participation Fees (checks made payable to Timothy Edwards Middle School):
  •  $100 per sport with a $400 family cap


Registration Deadlines

In order to participate on the first day of tryouts, all parts of the the online registration process must be complete, and the Physician's Statement and Sports Participation Fee must be turned in to TEMS main office - no exceptions!

Tryouts will begin on Tuesday, September 3, 2019. Details will be provided at a later date.
